1. Patient is anxious, agitated, or restless.

2. Patient is co-operative, oriented, and calm.

3. Patient is responsive to verbal command only.

4. Patient exhibiting brisk response to light glabellar tap or to an auditory stimulus.

5. Patient exhibiting a sluggish response to light glabellar tap or to an auditory stimulus.

6. No response to any of these stimulations.
